First Visit Guide

When you come for your appointments, please remember to bring the following

• Your completed paperwork/registration forms posted to you prior to your appointment.
• Your original referral from your doctor (GP or Specialist). To be eligible to claim your rebate from Medicare you MUST have a valid referral letter. A referral from a GP is valid for 12 months (unless otherwise stated) and a referral from a specialist to specialist is valid for 3 months.
• Payment – which is required on the day of consultation
• X-rays, MRIs, CTs, scans relating to your condition. (FILMS PREFERRED and any associated reports) Copies of operation records, Medical records that may be relevant.

Very often during the course of your evaluation or treatment for an orthopaedic condition we will require x-rays to determine the cause of a problem or to evaluate your progress.
